A certificate of insurance is required before use of any space (if not SWMetro employee). Please include SouthWest Metro Intermediate District 288 as additional insured. I understand the behavior expectations for all persons in this facility and acknowledge that my group may be asked to leave the facility and will forfeit all monies paid if any participants fail to follow the code of conduct for this facility. The administration reserves the right to permanently ban any group or organization from this facility that fails to comport to the conduct standards of the facility.
